COVID 19 Health Declaration Form – August 2021

Dear Parents/Guardians,

We are very much looking forward to welcoming your child back to the new school year next week.

To prepare for your child’s return, it is very important that parents/guardians complete the ESF COVID 19 Health Declaration by Sunday 15th August letting us know that your child is safe to return to school.  Without a completed declaration he/she will not be able to enter the campus. Please access the COVID 19 Health Declaration via the SMART VLE button on the ESF App or through the SMART webpage to complete the declaration.
