Travel & Health Declaration Form – January 2021

Dear Parents/Guardians

As you are aware, ESF schools will commence a limited return to campus for your child(ren) starting next week. Please be assured that all of our cleaning and disinfection protocols are in place as well as strict social distancing and safety measures, to ensure the safety of all students and staff.

For those with children returning to school on or after 11th January 2021, you must complete the Travel and Health Declaration before 12 noon this Saturday, 9th January 2021. Your child(ren) will not be able to enter campus until the declaration is completed and submitted. Please go to the SMART VLE button within the App to complete the form.

For those children with no confirmed dates to return to school, please do not submit the form.

On the days your child(ren) will be on campus, please complete the daily temperature check on the App each morning.
