Student Health Service

Dear Parents and Guardians,

The Student Health Service of Department of Health is now conducting its annual enrolment exercise for all primary and secondary school students for 2015/2016. They would like to invite all children to participate.

Please check the letter and application form that will be sent home with your child(ren) today for further information. I am kindly requesting all applications (regardless of intent to participate) be completed and returned before this Friday (September 11). There is a drop off box in the Medical Room (Rm 104).

Families interested in participating will receive followup information and an appointment date and time. Those not interested in the program need not participate, however, they must also complete the first section on the application form and return it to either the main office/Reception or the Medical Room.
